I don't know if there is a "best way," as there are several ways to get good results. Kyle gave you one, and it sounds like you have seen a few others.
Below is a link to the way I put mine together. It doesn't have a stream, only a series of waterfalls that trickles into a small pond/bog that is now overrun with Cryptocoryne wendetii. I hope it helps.
