Actually, that reminds me that I should provide a little context to that part :-) I used to use NE Herp (it was great, I just decided to try something I could get locally rather than having to ship all the time) and that is where I got used to adding vinegar and yeast. I just kept doing it with the new medium, even though the directions didn't say to do so. I have had better luck since starting to follow the directions of my new medium to the letter. So, my situation isn't quite the same as yours.

From your picture, is that a really dense and space-filling wad of excelsior in there? Any chance that is blocking off air flow from the bottom part of the culture? Is the culture in a really humid room? You could also try to cut back a little bit on the amount of water you put in the culture to begin with. If your cultures are producing well, I wouldn't worry about it. As SM said, you can always do some experimentation, too.
