I think my plant is dieing..

So, I've had this plant for a while and it was putting off new growth and growing well untill 3 weeks ago it yellowed and now it really looks like it's dieing. I'm debating taking it out to try and put it in a fidderent container and hopefully save it. Is this a good idea? I'm starting to feel that it's roots are getting choked out..

It looks to me like you are keeping your medium way too wet.
